For what purpose do we collect personal and sensitive information?
Your participation in this survey aims to capture your insights and informs the advice regarding the distribution and placement of GP registrars in Queensland as part of the Australian General Practice Training Workforce Planning and Prioritisation (GP WPP) program. Advice and analysis specifically relate to the workforce needs of communities, in addition to the training capacity of practices within GP Catchments in Queensland.
We collect personal and sensitive information about you to fulfill our functions, responsibilities under and to facilitate compliance with the delivery of the GP WPP program and broader PHN Program, as required by the Department of Health and Aged Care, including:
performing analysis of the workforce and/or training capacity needs.
building an informed understanding of circumstances relating to the needs of community and general practice workforce.
development of a Queensland Workforce Needs and Training Capacity Report, and Queensland Training Pathways Report.
Taking part in this survey is voluntary.
What type of information do we collect?
We only collect personal or sensitive information about you that is reasonably necessary for or directly related to our functions and activities, and relevant to the purpose for which it is collected. We will only collect information with your consent, or as otherwise authorised or required by law.
Depending on your role, we collect the following information:
Personal information: Name, facility/practice name, contact details and age.
Sensitive information: Supervision status, opinions, and experiences regarding GP registrar supervision.
By participating in this survey and providing responses, you are disclosing personal and sensitive information to the GP WPP program Queensland Primary Health Network Consortium (we/us/our), led by Brisbane North PHN (ABN 55 150 102 257), in accordance with obligations under the Privacy Act 1988 (Cth), the Australian Privacy Principles and any registered privacy code that applies to us.
We will take reasonable steps to de-identify personal and sensitive information before using or disclosing it for analysis or reporting purposes. De-identification means removing personal identifiers such as name, address, or other information that could be used to identify you.
Storage and Security
We take reasonable steps to ensure the personal and sensitive information we hold about you is protected from misuse, interference, loss, unauthorised access, modification, or disclosure. We store personal and sensitive information securely in electronic and/or hard copy formats. Personal and sensitive information is accessible only by authorised personnel and only for the purposes for which it was collected.
We keep personal information for as long as it is required for our functions and activities or as required by law. Once the personal information is no longer needed, we take reasonable steps to destroy or de-identify it.
Disclosure of Information
Any information that is disclosed will be used only for the purposes stated in this notice and will be de-identified. We will also aggregate the information we collect from participants for reporting and analysis purposes. Aggregation means that we combine and summarise the information so that it no longer identifies any individual participant.
We may share the aggregated data with third parties, including:
The Royal Australian College of General Practitioners (RACGP) and the Australian College of Rural and Remote Medicine (ACRRM).
Government agencies, Department, and entities (such as Department of Health and Aged Care, Rural Workforce Agencies, State and Territory health jurisdictions).
Other Workforce Planning and Prioritisation Organisations.
However, we will take reasonable steps to ensure that the aggregated data cannot be used to identify any individual participant.
